Seeking program
manager to provide comprehensive clinical and administrative oversight and
management of the SAMHSA project at Safe Recovery. This position will provide
daily oversight to the project work, as well as supervision for the MAT
Services Navigators and Case Managers. The Program Manager will be responsible
for internal and external coordination with stakeholders, and will also take
the lead to assure timely reporting to the sponsor. Further, they will direct
activities of all contractors. This position will provide care coordination for
clients receiving buprenorphine treatment through the SAMHSA project. They will
provide individual and group psychotherapy and support services to clients
across the lifespan. This includes conducting assessments, developing treatment
plans.

Responsibilities:

* Work

with Program Coordinator and Buprenorphine Provider on program design and

implementation.

* Recruit,

hire, train and supervise Navigators and Case Managers.

* Direct

activities of contractors.

* Coordinate

evaluation with the grant evaluator and research assistants.

* Coordinate

with the syringe exchange staff on client recruitment and engagement.

* Engage

with community MAT providers to improve coordination and collaboration.

* Participate

in internal and community meetings and prepare written reports.

* Coordinate

and provide access to preventive and health promotion services, including

prevention of mental illness and substance use disorders.

* Coordinate

and provide access to mental health and substance abuse services.

* Coordinate

and provide access to comprehensive care management, care coordination,

and transitional care across settings. Transitional care includes

appropriate follow-up from inpatient to other settings, such as

participation in discharge planning and facilitating transfer from a

pediatric to an adult system of health care.

* Coordinate

and provide access to chronic disease management, including

self-management support to individuals and their families. Coordinate and

provide access to long-term care supports and services.

* Coordinate

and provide access to individual and family supports, including referral

to community, social support, and recovery services.

* Conduct

assessments and develop a person-centered treatment that coordinates and

integrates all of client's clinical any non-clinical health-care related

needs and services.

* Demonstrate

a capacity to use health information technology to link services, facilitate

communication among team members and between the health team an individual

and family caregivers, and provide feedback to practices, as feasible and

appropriate.

* Ensure

documentation from engaged providers of care is appropriately noted.

Requirements: 

* Masters

in related field

* Roster

of Non-Licensed & Noncertified Psychotherapists (See Licensing

Requirement Notice below)

* Two

years working in substance use field providing counseling and case

management..

* Working

knowledge and application of medication assisted treatment and harm

reduction principals, supervisory skills, interpersonal skills, clinical

skills

* Computer

skills including use of electronic medical record,

* Valid driver's license, registered vehicle and automobile insurance that complies with agency policies

* Ability

to engage clients, staff and community partners, organize, collaborate,

multitask, budget, coordinate care, provide counseling, maintain accurate

and complete clinical and administrative documentation, compile and

analyze data, write reports.

* Strong

understanding of systems of care in Vermont, harm reduction principles,

medication assisted treatment, trauma informed care.

Licensing Requirement
Notice

Vermont regulation
mandates that if you do not hold a valid clinical license then you must
added to the Roster of Nonlicensed & Noncertified Psychotherapists
before you can be hired for work in this position. If you are not yet on the
Roster, please begin the process immediately to avoid delay in hiring. Note
this process may take 3-5 days for in-state applications and up to several
weeks for out-of-state applications. Instructions can be found on the website
for the Vermont Office of Professional Regulation.

Desired:

* LADC,

LCMHC or LCSW preferred.

* Program

management experience preferred
