The Home and Community Based Therapist (HCBT) provides intensive, short-term (6 to 9 months), home-based therapy and case management to families in the community. The HCBT will support families to develop a treatment plan and identify and implement therapeutic interventions to support families in reaching established goals. He or she will also act as the clinical lead for their clients’ treatment teams, participate in consultation with other Children, Youth and Family Services (CYFS) clinicians, and may provide other therapeutic services as determined by supervisors.
